To begin working with Nationwide Security Services all you need to do is download our Screening Agreement and Membership Application, complete and sign both, and fax them to 631-794-2453. We will take it from there.

If you wish to include Credit Reports in addition to our other screening services, the applicable laws require us to obtain additional documentation. This includes a letter of intent on your company letterhead with supporting corporate documentation, and signed copies of a completed Access Security Requirements form and Trans Union Addendum form.
